Scheduling Instructions

  1. Visit our website at MainlinePharmacy.com
  2. Under the “COVID-19” tab, click on “COVID-19 Vaccine Scheduling”
  3. You will then be prompted to enter the password you were given on the phone call.  (Please do not share this password.  Anyone not on the call list will not be given the vaccination at the clinic, regardless of if they were able to make an appointment)
  4. If there are times available, click on the time you would like to reserve
  5. Click Continue
  6. Enter your information in the required fields
  7. Click complete appointment
  8. You will be taken to a page confirming your appointment
  9. Please print the consent form out prior to your appointment, fill it out, and bring it with you to your appointment. The form can be found here.
